Urination is the process of emptying the bladder through the urethra that connects the urinary bladder to the external urethral o. There are two sphincters, or valves, that keep the urethra closed to prevent leak: internal urethral sphincter located at the neck of the bladder, and external sphincter located below the prostate gland and is supported by the pelvic floor muscles. When the bladder is full, stretch receptors in the wall of the bladder send a signal to the spinal cord and the brain. At times when it's not appropriate to urinate, the brain sends back an inhibitory signal to keep the sphincters closed and prevent voiding. When you wish to urinate, this inhibition is removed; the spinal cord instructs the muscle of the bladder to contract and the sphincters to open to let the urine out. Urinary incontinence is involuntary leakage of urine. There are several types of urinary incontinence in men: 1. Stress incontinence: urine leakage while sneezing, coughing, laughing or any activity that creates abdominal pressure on the bladder. This usually occurs because the muscles or tissues underneath the bladder are weakened and can no longer support it. In men, this typically happens after the prostate gland is removed for reasons such as prostate cancer. 2. Urge incontinence: need to void that cannot be deferred, inability to hold. This is commonly caused by an overactive bladder, a condition in which muscles in the wall of the bladder contract in an uncontrollable manner. The reason why this happens is unclear but it's likely to involve problems in the nervous system. 3. Overflow incontinence: constant dribbling of urine. This happens when the bladder does not empty properly while voiding making it almost always full and urine overflows. This may be due to weakened muscles in the bladder wall or a blocked or narrowed urethra. In men, the major cause for this type of incontinence is a condition called enlarged prostate or benign prostate hyperplasia, where the abnormal growth of prostate tissue compresses the urethra and blocks urine flow. Treatment depends on the type of incontinence and severity of symptoms. 1. Lifestyle changes - Limit fluid intake at certain times of the day - such as before going to bed or before a long trip. However, note should be taken to increase fiber content in your meals to prevent constipation. - Cut down on caffeine, alcohol, keep a healthy weight. - Try pelvic floor muscle exercises such as Kegel exercises. This is to strengthen the muscles that support your bladder. - Timed voiding or bladder training therapy: plan regular trips to the bathroom at set times of the day, gradually increase the interval between trips as you gain control. 2. Medication Depending on the cause of incontinence the following types of drugs maybe prescribed: - Alpha-blockers, 5-alpha reductase inhibitors: for treatment of enlarged prostate. - Imipramine, antispasmodics : these act on nerves to block bladder spasms in an overactive bladder. 3. Surgery Surgical procedures include: - Implantation of artificial sphincter: this is performed when weak sphincter is the source of the problem. - Male sling: implementation of an artificial support for the urethra. This is usually recommended for those who have had their prostate gland removed. - Other surgical procedures for treatment of enlarged prostate.

Our Artificial Urinary Sphincter ZSI 375 treats severe urinary incontinence in men. The urine can flow out of the bladder. Few minutes after the bladder is empty, the cuff is automatically refilled, and the urethra is squeezed closed again. The principal design features of the ZSI 375 include the following: - One-piece assembly to facilitate implantation and minimise mechanical failures - Logistic of delivery simplified thanks to an adjustable cuff that suits all sizes of urethra. - A cuff moulded in circular form to reduce potential stress fractures caused by creasing - A simple and rapid procedure Synthesis of our sphincter added value. Repeat 8-12 Kegel Exercises, 3 times daily holding each exercise for 3-10 seconds. B. Technique for Short Strong Kegel Exercises *Sit with tall posture *Squeeze and lift the muscles in and around your 3 pelvic openings with a quick brisk action *Lower your pelvic floor muscles back to starting position *Repeat up to 10 short Kegel exercises in a row Start out repeating this exercise for that number of contractions that you can manage with the correct technique. Practice these short contractions daily. Try to increase the strength of the contractions as you pelvic floor strength improves. Kegel Exercises for Bladder Control Use these quick contractions before and during every cough, sneeze or laugh. You need to contract your pelvic floor muscles before and during the activities that place pressure on your bladder causing bladder leakage. Relax your pelvic floor muscles back to resting when you are not doing your exercises. Avoid walking or running with your pelvic floor muscles contracted as this practice can cause pelvic floor muscle tension and pelvic pain. Men can experience bladder leakage or urinary incontinence for a number of different reasons. What happens is the sphincter in the bladder does not function the way that is should. When the kidneys filter the blood they produce urine that is stored in the bladder. The sphincter's job is to hold the urine and release it when the brain tells it to. When the brain cannot communicate with it, this causes bladder problems. The sphincter can remain closed at all time not allowing urine to exit. It can remain open at all times which can cause incontinence, or it can open and close on its own without any warning. Investigations to perform are postvoid residual volume measure amount of urine in bladder after voiding by ultrasound, catheter. Normal is less than 50mL and greater than 200mL is diagnostic. Anything in the middle is gray area. Treatment is to treat underlying cause if obstructive. Non-obstructive can be treated with Chronic Intermittent catheterization. NEUROPATHIC INCONTINENCE Due to lesions in brain, spine or peripheral nerve. Caused by strokes, trauma, aneurysms, hemmorrhage, MS. Spinal cord lesion and cauda syndrome. May cause decrease compliance by have high bladder pressure at low filling rates causing quick leaking. Common in meninomyelocele and Upper motor Neurons. Loss of sphincter tone by cauda equina, and open external sphincter. Investigations consists of neurologic, urodynamic, CT, MRI, and EMG. Treatment consists of increasing compliance with anti-cholinergics oxybutinin, imipramine, botulinum. Sphincter can be used with pessary to block the sphincter and when ready urinate the cork can be removed.
